 Application Form
The first step toward living on campus is to submit application for placement in the U-Mary residence life program. A $100 deposit and a completed Roommate Finder Questionnaire must be submitted with the application. The Admissions Office sends application forms to all new students upon their acceptance at the university. Current students may pick up a copy of the Application Form at the Student Development Office located in Room 1209 of the Benedictine Center for Servant Leadership. 

 Roommate Finder Questionnaire
Are you looking for a roommate who keeps the room tidy, or would you prefer the "lived in" look? The Roommate Finder Questionnaire provides students an opportunity to express a number of preferences that the university uses to selecting the most compatible roommate possible. All students applying for on-campus housing are asked to submit a completed questionnaire (students applying for a single room should also complete the Roommate Finder Questionnaire in case the university is not able to accommodate the request for a single room). The form is available for downloading at the following link:

 U-Mary Residence Life Agreement -
The U-Mary Residence Life Agreement outlines the terms and conditions by which the university grants a student a license to participate in its on-campus residence life program. Each student is required to sign the agreement before moving into his/her on-campus student residence.

 Form to Request for Permission to Live Off Campus - The University of Mary requires traditional-age freshmen and sophomores to live on campus. Exceptions require approval from the vice president for student development using the following form:

 Form to Request Early Cancelation of Residence Life Agreement
Students who would like to cancel their residence life agreement with the university before the term of the agreement expires should familiarize themselves with the policy for cancelation in the agreement. Penalties may apply. Requests for cancelation should be submitted using the following form:
